• Basic Benefits: Hospital confinement daily benefit is sold in increments of $10 starting at $30 a day to a maximum of $250 a day. This plan does not pay in addition to Workers Compensation Benefits.

- Annual Wellness Benefit Rider: Provides benefits for annual physical exam up to $100, flu shot up to $20, annual eye exam up to $40, prescription lenses and frames up to $50, annual hearing exam up to $40 and purchase of a hearing aid up to $100, semi-annual routine dental visits up to $25.
- Family Rider: Pays a daily benefit for hospital confinement due to covered accident or sickness

Benefit paid as one-time, lump sum if the insured undergoes a covered organ transplant or is first diagnosed with a heart attack, stroke, life-threatening cancer, or kidney failure. Cancer benefit is optional.
Diagnosis of terminal illness not required.
Spouse may apply for coverage if the employee does not apply for coverage.
Benefit paid in addition to any other coverage insured may have.
Benefits reduce to 65% of original benefit at age 70, 45% at 75, 30% at 80 and 15% at 85.

Portability available at group rates
